Key Responsibilities:
- Complete necessary forms, reports, evaluations, and studies related to social services
- Assist with resident admissions and develop post-discharge plans
- Maintain therapeutic relationships with residents and facilitate regular communication with their families
- Coordinate social services activities and contribute to quality assurance programs
- Process abuse and grievance complaints, working with local agencies and keeping the administrator informed
- Participate in care plan conferences and provide counseling to residents, families, and staff
- Retain knowledge of community resources to meet the comprehensive care needs of residents
- Assist with pre-admission screenings and support the overall care planning process
- Bachelors degree in Social Work (BSW) or related field
- Active LSW or LCSW licensure in good standing, or Masters degree in Social Work
- Flexible Schedule: Autonomy over start/end times
