Are you a quick thinker, organized, reliable, and caring? We are seeking a dedicated Social Worker with these qualities to join our team at Anchor Care and Rehabilitation Center.

In this role, you will collaborate with the Director to assist in planning, organizing, and implementing the overall operations of our Social Services Department.

Salary: Up to $30 per hour

Responsibilities:

• Conduct interviews with residents and families to gather social history.
• Engage residents and families in the planning of social service programs whenever possible.
• Provide solutions for social and environmental challenges, including financial assistance, discharge planning, and referrals to specialized community agencies.
• Collaborate with administration, medical, and nursing staff, as well as other departments, to plan social service programs and activities.
• Participate in facility surveys and inspections conducted by authorized government agencies.
• Communicate residents' social, psychological, and emotional needs to the medical staff, attending physicians, and other care team members.

Requirements:

• Minimum of 1-year experience working in a Skilled Nursing Facility.
• Bachelor’s or Master’s degree in Social Work (BA or MSW).
