Roseburg is located on Interstate 5 in Southern Oregon's scenic Douglas County. The community is nestled in a lush forested region known as the 100 Valleys of the Umpqua. Responsibilities:Shift: Casual Part-Time, Weekdays
Pay Scale: LCSW $36.87 - $53.46; MSW $33.52 - $48.60
Job Summary:
Conducts psychosocial assessments, determines patient and family needs, and develops a plan of treatment consistent with meeting those needs. Acts as a consultant to other disciplines. Interprets psychosocial information to patient, families and other professionals and works to link patients and their family to community support systems.
Essential Duties:
- Conducts psychosocial assessments on referred patients and develops a plan of treatment consistent with standards established under Medicare or other third party payors.
- Provides support and crisis counseling to patients and family members.
- Maintains a current and complete knowledge of local, state, and federal agencies and resources. Acts as a liaison between these agencies and staff/patients and completes referrals as appropriate.
- Responsible for coordinating and implementing discharge plan for specific patients as appropriate.
- Provides consultation services to other disciplines, evaluating and interpreting psychosocial information to other professionals.
- Completes documentation as required by Medicare, other third party payors and The Joint Commission.

Qualifications:License/Certification:
- LCSW or LMSW licensure from the State of Oregon
Experience:
- Healthcare experience preferred
Education:
- Master’s degree from school of social work accredited by the Council of Social Work Education.
