Sigma Healthcare team is looking for an experienced Social Worker who will help to oversee the actions of our current case managers that provide many services already to our patients.

Summary:
- The Social Worker will see patients as well as oversee the case manager's work as it pertains to social work, review and sign off on the progress notes that have been created. As well and provide insight into opportunities for our patients to gain better access to programs that will help them.
Responsibilities:
- Provides and create direction, supervision, and evaluation of medical social services to patients.
- Oversee the progress notes of case managers and outreach who work directly with the patients to ensure progress note is ready to be processed as an encounter.
- participates in the coordination of SWING and Skilled Nursing Facility admissions, discharges and Utilization Review.
- teaches in-service stafftrainingon Social Services related topics.
- Design and implement individualized care plans based on patient's needs and goals as provided by Mental Health providers and case managers.
- Coordinates facility participation in telehealth programs.
- Assists in grant writing.
- Consults and coordinates care with primary care physicians, psychiatrists, nurse practitioners, case managers, and other health care providers to ensure the best care for patients.
- Assist and inform patients in accessing community resources and health/social service benefit programs that are available to them.
- Participate in staff and treatment planning meetings.
- Assess patient safety and provide crisis intervention for patients as needed.
- Participate in monthly agency Quality Assurance and Provider meetings and interventions as needed.
- Works under the general supervision of the Psychiatrist and Clinic Administrator.
- Abides by and demonstrates the CIMC Mission Vision Values through both behavior and job performance on a day-to-day basis.
- Current and valid Clinical Social Worker's license (Required).
- Bilingual English/Spanish preferred but not required.
- At least 1 year of experience in the field
- Cultural sensitivity.
- Have a team player attitude and ability to work well within a close-knit team setting.
