We are currently looking for a Social Worker to join our team! The ideal candidate is someone who has a Master of Social Work (MSW) and group work experience.
Job description:
Provides social work services to patients of the CT Behavioral Health Hospital with regard to problems of illness, aging, and adjustment to lifestyle changes brought about by health, aging and changing family relationships within the course of the life cycle. Partners with Social Workers and other healthcare professionals to identify appropriate utilization of resources, ensure timely and accurate reimbursement, create comprehensive discharge plans in accordance with patient needs, and educate both patients and families.
Key Responsibilities:
- Participates in formal admission screening, as required by the CT Behavioral Health Hospitalcriteria.
- Psychosocial assessment on all new admissions within 24 hours / prior to psychiatrist evaluation.
- Attends probate court hearings. Prepares and files papers required for probate hearings.
- Assesses, in cooperation with treatment plan patient's social and emotional status regarding illness or disability and adjustment to the facility.
- Assists in establishing short- and long-term goals towards reaching and maintaining optimal levels of function and documents progress on the Master Treatment Plan.
- Participates in multi-discipline team meetings on the CT Behavioral Health Hospital unit two (2) times weekly basis, discuss, update and present new admissions.
- Assists in patient adjustment to the facility providing support and appropriate intervention during periods of crisis and individual social work case services to both patients and family members as required and in cooperation with Treatment Team.
- Arranges family conferences as needed, to discuss issues surrounding patient's care.
- Run groups 2-3 times per week and document in group notes section of chart.
- Initiates discharge planning from the day of admission in tandem with treatment recommendations for safe discharge planning. Works with appropriate Health Care community agencies when patient is discharged back to the community or another facility.
- Documents in the patient's medical record the nature, level and content of social work services. Routinely documents crisis intervention and other interim services. Completes all other required documentation and forms prior to admission and throughout placements.
- As appropriate leads inter-departmental in-service programs as requested. Participates in appropriate external conferences and workshops.
- Is supervised in casework and group work as needed by the Director of Nursingas required and/or as needed.
- Attends staff meetings as scheduled.
- Maintains effective relationships within department and other department staff.
- May be required to attend meetings and carry out other department functions, as designated by the department head.
- Assists the senior staff in carrying out any functions needed on the CT Behavioral Health unit.
Qualifications:
- MSW or MA from an accredited college or university, one-year previous casework and group work experience preferred.
- Ability to deal with people under varying conditions; ability to maintain effective working relationships with staff.
- Some supervisory experience preferred.
Working Conditions:
- Well-lighted, well-ventilated office.
- Sits, stands, walks, bends and stoops throughout the workday.
- Subject to frequent interruptions.
