Coordinates and advocates a caseload to guarantee high-quality programming and regulatory compliance for individuals with chronic mental illness for the Horizons PROS program.

Responsibilities:

• Facilitates groups and provides group coverage in the absence of other staff.
• Completes all required OMH regulatory documentation for billing within program guidelines.
• Acts as liaison with families and outside agencies, ensuring continuity of services
• Responsible for coordination of individual’s care through the recovery process.
• Provides intake and individual counseling as needed.
• Assists their caseload in developing their Individual Action Plans and defining recovery-oriented goals.
• Assists their caseload in crisis using their Relapse Prevention Plan.
• Monitors and documents their caseload’s weekly High-Risk Plans and AOT/MHC orders

Qualifications

• One year of experience with the chronic psychiatric population/three years experience is preferred.
• Maintains a valid NYS driver’s license within hospital criteria.
• Master’s degree in Social Work/NYS License required.
