Social Worker (LMSW/LCSW) - Tandym

Mental health facility is seeking a Social Worker to join their team in the Outpatient Department.

Responsibilities included but not limited to are:

• Provides new patient orientation re: Outpatient services
• Completes intake evaluations
• Accurate and timely completion of Social History (Core History) via EMR system
• Completes Treatment Plans (with appropriate Goals and Objectives) taking into account cultural/spiritual issues, as well as treatment issues for the older individual, when appropriate.
• Engages patients in a therapeutic relationship, to provide rehabilitative and treatment interventions and assist to access entitlements as needed (at least monthly contacts).
• Monitors for signs of substance abuse and intervenes for appropriate care and treatment where possible.
• Reaches out to work collaboratively with the Outpatient psychiatrist, patient's families, significant others and other supports and agencies
• Leads structured effective treatment groups.
• Provides crisis intervention.
• Records contacts and assessed treatment programs (using appropriate coding) in Progress notes in the electronic record

Requirements

• Active and unrestricted New York State Licensed Master of Social Work (LMSW) or Licensed Clinical Social Work (LCSW) license
• Prior experience working in a mental health or social service setting.
