Amberwoods is a 130-bed facility providing long-term skilled nursing, short-term rehabilitation and Memory Care services, located in Farmington, CT. Our facility is conveniently located within proximity to I-84 and is a ten-minute drive to downtown Hartford.

- Demonstrates patience, a positive attitude, and the ability to be a team player.

- Completes required EMR documentation in a timely manner including assessments.

- Holds Resident Care Plan Conferences with the interdisciplinary team to review resident's plan of care.

- Assists the interdisciplinary team with any concerns that arise with patients and/or families.

- Support residents' psychosocial needs.

- Support residents, responsible parties and families during adjustment to placement.

- Maintains timely documentation of above.

Experience:

- Nursing Home experience preferred.

- Knowledge of the PASRR process and the federal/state rules and regulations preferred.

Education Requirements:

Bachelors or Master’s degree in Social Work.

Pay: Up to $65,000.00 per year
