Work Type: Full Time (Total FTE between 0.9 and 1.0)
Shift: Shift 1
Work Schedule: 8 Hr (8:00:00 AM - 5:00:00 PM)
SUIT Inpatient Social Work Role – General Expected Duties
1. Independently initiate and complete clinical and case management social work encounters with patients, with an emphasis on supportive counseling/Motivational Interviewing.
+ Assessments can include, but aren’t limited to:
+ Comprehensive psychosocial assessment (psychosocial history, substance use history, etc)
+ Exploration of stages of change
+ Motivational Interviewing discussion around patient’s ideas for change, goals for recovery, harm reduction
+ Agenda mapping and prioritization of next steps
+ Education about what substance use treatment is, what to expect.
+ Eliciting and troubleshooting worries, questions about treatment.
+ Introduction to the idea of MAT in medically appropriate patients.
2. Manage all aspects of substance abuse related discharge planning
+ Help create complete aftercare plan, which may include:
+ Appointment scheduling
+ Communicating pertinent information to outpatient providers, particularly SUIT Clinic.
+ Coordinating as needed with inpatient case manager and other specialty teams
+ Coordinating across multiple community-based organizations.
3. Collaborate with multi-disciplinary SUIT team and provide your clinical assessment of patient.
+ Participate in daily interdisciplinary rounds
+ Communicate anticipated barriers with SUIT clinic team for new or returning patients.
4. Role with Students, Residents and Fellows
+ Trainees are guest members of the SUIT team who are here to learn an interdisciplinary model of care
+ You may be asked to participate in the general education of trainees.
5. Data Collection
· Participate in data collection and other grant related projects that support SUIT.
Other information: Required Job Qualifications:
• Master's degree in Social Work.
• Current LSW licensure in the State of Illinois.
• Two years of experience providing social services in healthcare or community setting
• Experience utilizing motivational interviewing and/or in a managed care environment.
• Strong verbal and written communication skills, analytical skills and team-building skills.
• Ability to function independently, to organize and be self-motivated.
• Ability to sense problems or potential problems.
• Ability to apply inductive and deductive reasoning to information received and identifying relationships between seemingly unrelated events/information.
Preferred Job Qualifications:
• Current LCSW licensure in the State of Illinois.
• Behavioral health experience.
• Familiarity with electronic medical records.
