Social Worker-Maternal Child Health - Ascension

We Are Hiring:

• Department: Inpatient
• Schedule: Monday-Friday, 8:00AM-4.30PM, with rotating remote on-call from 5:00PM-10:00PM
• Hospital: Ascension St. Agnes
• Location: Baltimore, MD
• Salary: $29.20-$43.80

Actual compensation offered will vary based on factors including applicable licenses, certifications, education, experience, location, qualifications and comparison with associates in similar roles.

 

LMSW, with 2 years comparable acute experience working with children and families, Maternal Child Health, hospital, and/or community setting

Experience with complex medical patients is a plus

What You Will Do:

Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

• Create plan for care across the continuum, integrating patient/family preferences and values.
• Serve as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
• Advocate for resources and removal of barriers.
• Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.
• Act as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care.

What You Will Need:

Licensure / Certification / Registration:

• One or more of the following required:
  • Clinical Counselor credentialed from the Maryland Board of Professional Counselors and Therapists obtained prior to hire date or job transfer date.
  • Counselor credentialed from the Maryland Board of Professional Counselors and Therapists obtained prior to hire date or job transfer date.
  • Counselor obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date. Any level accepted.
  • Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
• Case Manager credentialed from the American Case Management Association (ACMA) preferred.

Education:

• Bachelor's degree in Social Work required.

Additional Preferences:

Work Experience:

• 3 years of experience preferred.

We are seeking a dedicated and experienced Maternal Child Health Social Worker to join our team at Saint Agnes Hospital. The ideal candidate will be responsible for providing comprehensive maternal and child health services, ensuring the highest quality of care for mothers and their children. This role requires collaboration with healthcare providers, community organizations, and families to promote health education, preventive care, and effective health management strategies.

• Provide direct patient care to mothers and children in accordance with established protocols and standards.
• Conduct assessments of maternal and child health needs, to identify barriers to treatment and encourage compliance with medical care
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care for patients.
• Educate families about prenatal care, infant care, safe sleep, and Post Partum Mental Health
  will be provided the opportunity to Participate in community outreach programs aimed at promoting maternal and child health awareness.
• Maintain accurate records of patient interactions and outcomes in compliance with hospital policies.

Excellent communication skills with the ability to engage effectively with diverse populations

• Social Worker credentialed from the Maryland Board of Social Work Examiners. Any level accepted.

Preferred Credential(s):

• Drivers License credentialed from the Maryland Department of Motor Vehicles.
