Job Summary and Qualifications
The medical social worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitatestimely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses and tracks barriers to patient throughput.
What you will do in this role:
- You will perform continuous assessments and evaluations to ensure patient is progressing towards desired outcomes.
- You will collaborate and consult with physicians on patient's progress and discharge planning needs.
- You will perform utilization review ensuring admissions meet criteria for appropriateness of care and medical necessity.
- You will coordinate the discharge planning process ensuring involvement of all members of the healthcare team. Counsels with patients and family members in decision making and in meeting psycho social needs of the patient.
- You will collaborate with personnel at other facilities to coordinate smooth and effective patient transfers and transitions.
What qualifications you will need:
- State of TN LCSW Licensure preferred.
- Bachelor's Degree in Social Work requiredMaster's Degree preferred.
- 1 year in Healthcare with demonstrated understanding of acute care social service issues
Parkridge Medical Center is a 270+ bed hospital. We are near downtown Chattanooga. We feature comprehensive diagnostic, medical, and surgical services. These services include cardiovascular surgery and cancer treatment. These services include intensive care, orthopedic and spine surgery, and rehabilitation. The facility includes a state of the art neurosurgery program. Patients also have access to OB-GYN services. Patients have access to a level III Neonatal Intensive Care Unit. They also have access to (NICU) and behavioral health treatment centers for children and adults.
