Atlantic Health System is seeking a per diem Social Worker for Case Management Department at Overlook Medical Center. This position is per diem with flexible hours including both weekday and weekend/Saturday coverage needed.
- Consultant to healthcare team
- Resolution of crucial problems of patient/family
- Assist with adjustment to illness issues
- Facilitate formulation of post-acute care transition plan
