Social Worker - Kingston Healthcare

Kingston Healthcare is seeking a compassionate and dedicated Social Worker to join our team of healthcare professionals. As a Social Worker at our facility, you will be responsible for providing social services to residents and their families to ensure their emotional and social well-being.

Responsibilities:

• Conduct psychosocial assessments of residents and develop individualized care plans.
• Provide counseling and support to residents and their families.
• Collaborate with the interdisciplinary team to coordinate care and services for residents.
• Assist residents in accessing community resources and services.
• Advocate for residents' rights and well-being.
• Maintain accurate and up-to-date documentation of all social work services provided.
• Develop, coordinate and assist with discharge planning
• Coordinate services in community, homecare or outpatient therapy
• Oversee care conferences to assist in developing discharge plans
• Complete social services assesment and ongoing documents in clinical record

Qualifications:

• Bachelor's or Master's degree in Social Work from an accredited program.
• Current Social Work license in the state of Ohio.
• Previous experience working in a healthcare or long-term care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team.
