Duration: 3 months with possible extension

Shift: Days 9 am to 5:30 pm

Willingness to work 1 weekend per month.

Overall Weekly Pay Rate: $2,320
W2 Weekly Pay Rate: $1,276
Weekly Stipend: $1,044.00

Job Description:

· Adhere to CMS required documentation of Important Message, social service initial and ongoing assessment, Patient Choice, Discharge disposition, Readmission Assessment, and social service referrals within policy.

· Assist with placement of patients to psychiatric facilities and provide resources.

· Attend family meetings, work with community agencies for placement.

· Assist ED Case Manager with psychiatric patients, as indicated.
