About the Job
Position Summary
Coordinates and implements Plan of Care provisions related to the bereavement needs of surviving family members which will include, but is not limited to, phone calls, bereavement mailings, counseling, and memorial services. Assures the provision of quality professional bereavement care and programming, in accordance with the Medicare COPS and other relevant standards. Participates in IDG and serves as a resource to IDG regarding anticipatory grief issues as needed. Manages caseload of patients and families for grief support. Counsels patients and families in bereavement issues and makes appropriate referrals to outside agencies. Serves as a liaison to the community, other agencies, and/or individuals dealing with issues of loss and grief. Plans and coordinates hospice bereavement events. Maintains calendar of events.
Minimum Requirements
Education
- Masters – Social Work, Pastoral Care, or other relevant field of study.
Experience
- 1 year Hospice bereavement/counseling experience.
- 1 year event planning experience.
License/Registration/Certifications
- Valid Driver’s License
- Professional license (LMSW, LISW-CP, etc. OR minimum of 4 units CPE)
- Maintain CPR Certification

Core Job Responsibilities
- Adheres to all applicable federal, state and local regulations related to licensure.
- Provides timely bereavement support and counseling and insures that services and documentation are done within organizational and regulatory timeframes.
- Participates in On Call rotation and provides coverage for Outpatient and Hospice Home patients, as needed.
- Participates in / presents education programs about the Hospice programs through various community events.
- Participates in inter-disciplinary conferences and staff meetings.
- Assesses grief status and risk factors of bereaved and develops a plan of care based on identified needs.
- Documents all patient and family / caregiver interactions.
- Maintains and submits productivity as requested.
- Coordinates all bereavement mailings.
- Develops, acquires, and utilizes resources for bereavement support of families and persons in the community.
- Coordinates and/or facilitates support groups, memorial services, and other bereavement services.
- Executes the bereavement program as identified in organizational and regulatory policies.
- Attends events as required (weekend and after hour required).
- Public speaking required.
- Performs other duties as assigned.
