A Brief Overview
Provides professional pharmacy services. Reviews, interprets, monitors and dispenses medications, drugs and other pharmaceutical as prescribed by physician and according to professional standards and practices.
What you will do
- Prepares and dispenses medications as prescribed and ordered by physician/provider, including the calculation and preparation of solutions for oral, external, intramuscular and intravenous administration. • Comply with all applicable regulations and standards. • Receives physician/provider orders, preparing prescriptions and refill requests and intervenes when appropriate for clarification or correction. • Interprets and evaluates medication orders for contraindications, incompatibilities, allergies, and dosing. • Ensures patient profile is complete and adjusted accordingly for height, weight, current and past medical history, clinical laboratory results and concurrent medications (including over-the-counter and/or home medications) and to reflect provider’s desire to continue or discontinue said drug. • Checks dosage for appropriateness for neonatal, pediatric, adult and geriatric patients. • Accurately supplies medication to patients. Select, compounds and dispenses drugs and other pharmaceutical products. Verify accuracy of filled medication orders before dispensing. • Performs the preparation of drug labels and the affixing of labels to the proper containers; ensures the inclusion of appropriate directions for the timing, methods and precautions to be observed in taking the medication; personally instructs patients in the use of medications when indicated. • Prepares sterile and non-sterile drug products. • Orders routine and special medications. • Prepares and maintains records and other tasks normally associated with the operation of a Pharmacy in a clinical care context. • Ensures accurate documentation in performing and processing all orders/supplies to capture all charges and other relevant information. • Utilizes time in most effective and productive manner to ensure orders are delivered within established timeframes.
- Assists in the operation of assigned Pharmacy. • Checks the work of Technicians. Directs Technicians to perform tasks appropriate to current priorities and ensures all work is completed by the end of the shift. • Ensures routine and stat orders are processed within standards. • Assists in the maintenance of inventory and drug distribution systems including the maintenance of appropriate controls; notes and reports quality deficiencies of drug products; ensures that pharmaceuticals are properly ordered, stored, kept up to date and protected from deterioration. • Order supplies, rotate stock and return outdated medication/pharmaceuticals for credit. • Procures items from other pharmacies as needed. • Checks and restocks emergency boxes and Blue alert cart trays. • Complies with State, Federal and accreditation rules, regulations and standards. • Complies with, and enforces departmental, administrative, and medical policies and records. • Notifies Lead Pharmacist/Assistant Director of any issues or problems which can not be resolved or require intervention. • Recommends methods to improve Pharmacy services. • Participates in the quality improvement process.
- Provides clinical consultation. • Provides accurate drug information to physician, patients, and others about drugs and other pharmaceuticals with respect to such factors as the appropriateness of drugs in relation to patient’s condition, drug history, the choice of medications, dosage, strength, frequency and method of administration. • Consults on drug therapy, drug regimen review and drug utilization review. • Consults on drug storage and control. • Consults on research projects and other matters related to pharmacology. • Participates in medication use evaluations, records Pharmacist interventions, and adverse drug events. • Provides assessment, education and follow-up instruction for specified programs (i.e., Diabetes self-management, etc.).
- Operates computerized data systems. • Interprets and performs order entry functions in accordance with departmental policies and procedures. • Utilizes computer software applications to obtain drug information. • Operates computer and related equipment to enter and retrieve patient information. • Update patient information as required.
- Works effectively and serves as intermediary between patient care units and pharmacy; answers calls and provides information on matters related to Pharmacy Services. • Interacts and communicates in a customer responsive and courteous manner, in compliance with iRESPECT standards and promoting patient/customer satisfaction. • Provides information on drugs and pharmaceuticals to patients and medical/hospital staff in a professional manner. • Takes verbal orders from provider per policy and procedures. • Responds to Blue Alert Calls immediately. • Participates in drug research projects. • Works collaboratively with medical/hospital staff to coordinate and implement activities. • Provides in-services for staff and other departments.
- Participates in drug research projects. • Prepares investigational drugs products as needed. • Dispenses investigational drugs according to study procedures and protocols. • Completes all necessary documentation.
- Provides instruction, assigns and supervises other support/ancillary personnel assigned to unit to provide clinical assistance as needed. Assists in the orientation and oversee the supervision of new staff members and students when they provide patient care, treatment, and services as part of their training.
- Completes required continuous training and education, including department specific requirements. • Attends staff meetings, all mandatory in-service and additional continuing education as required. • Maintains professional credentials and education necessary to keep abreast and maintain a current knowledge of clinical/theoretical principles, concepts and techniques. Attends and actively participates in meetings, conferences, and seminars for the purpose of enhancing professional skills and sharing information on recent developments in fields of pharmacology and pharmaceutics. • Contributes to the annual review of unit-based scope statement and determination of important aspects of care for clinical review. • Assists in the development of indicators, thresholds, study methods, and data collection as assigned. • Responds to problems/opportunities to improve care. • Supports and is involved in the organization’s Performance Improvement initiatives.
- Maintains a clean, orderly, safe environment to minimize risk to patients and staff by complying with all safety/risk management policies and procedures. Adheres to established policies, compliance/quality improvement, and safety, environment and infection control standards. • Practices aseptic techniques according to policy and procedures. • Assists in care and maintenance of equipment and effective utilization of supplies. • Cleans and maintains all pharmacy related equipment. • Reports malfunction of equipment to supervisor.
- May be required to provide professional pharmacy services in a dual responsibility of clinical and distribution activities. Clinical activities include drug therapy monitoring, kinetics, and physician interaction in a variety of patient care areas.
- Provides research and administrative support to special projects, preparing data, reports, and follows-up on projects.
- Prepares and maintains required records, reports and files for administrative, operational and regulatory compliance purposes. • Documents all incidents which occur when designate pharmacist in charge. Makes copies of pertinent information. • Documents all interventions in computer system. • Documents participation of Blue Alert on intervention form. • Documents phone calls concerning nonformulary drugs on the intervention log. • Completes controlled drug records accurately. • Prepares reports of drug research projects and investigation drug findings.
- Demonstrates knowledge technical, interpersonal and critical thinking skills to provide care, maintain clinical competency and meet age-specific patient needs within pharmacy areas. Interprets and monitors appropriate drug therapies resulting from age specific groups. This includes the neonate, infant, adolescent, adult, and/or geriatric patient population as specialty assignment dictates.
- Provides coverage for relief and absences. Demonstrates flexibility to work night, weekend and holiday to provide continuous pharmacy coverage as required.
- Instructs/Counsels patients in the use of medications, assistance program, side effects, special precautions, missed doses, etc. when indicated.
- Contacts providers concerning prior authorization and medical necessity drugs for third party insurers and nonformulary medication orders.
- Process claims for payment.
- Checks patient identification and issues filled medication to outpatients.
- Provides assistance to customers/patient in the purchase of over-the-counter items.
- Perform cashier functions and collects payment from Medplex patients.
- Maintains knowledge of insurance plans, medication assistance program drugs and guidelines to promptly process prescriptions.
- Responds to customer/patient inquiries.
Qualifications
- Graduate of an accredited school of Pharmacy, M.S., and/or Pharm D Preferred and
- Qualified by education, training or experience to work with the neonate, infant, adolescent, adult, and/or geriatric patient population as specialty assignment dictates Required
- Tennessee Licensure as a Pharmacist Upon Hire Required and
- Additional certification may be required based on assignment Upon Hire Required
- Experience in patient care as obtained through completion of an approved pharmacy program Required
