Job Summary:
The Speech Language Pathologist is a member of patient care team within the Department of Home Health and directly accountable to the Patient Clinical Manager. Requires the individual be clinically competent and knowledgeable of home health regulations. Demonstrates knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs. The therapist assists in evaluating the level of function and helps develop the plan of care and make revisions as applicable. Prepares clinical and progress notes and advises and consults with the patient, family and other members of the health care team. Participates in in-service education and CEU’s as applicable. Performs specific therapy procedures in the home according to the plan of care. Prepares documentation of patient care in the home at the time of visit as applicable and submits documentation in a timely manner according to policy. Knowlegeable and compliant with 30 day patient Reassessments required by insurance. Notifies MD of any changes and instructs patient and family in meeting needs. Performs other job duties as assigned.
Performance Expectations:
- Demonstrates ability to safely perform and utilize Electrical Stimulation-Vital Stim equipment.
- Ability to select appropriate modes, timers, and inspect equipment before use and troubleshoot any issues.
- Demonstrate ability to enter all patients charting in EMR documentation meeting state and federal regulations.
- Must exhibit organizational skills that will allow for scheduling of all necessary visits for department.
Qualifications:
Education/Skills
Graduate of an accredited school of speech language pathology required.
Work Experience:
One year of clinical home health experience preferred.
Certification/Licensure-DUE UPON HIRE
- Licensed to practice Speech Therapy in the State of Mississippi.
- Valid Drivers License
Additional Certification/Licensure - Obtained based on required timeframe below
- Basic Life Support
