About the Job
We are looking for a skilled and compassionate Speech-Language Pathologist to join a home health team. In this role, you will provide essential speech-language therapy services to clients in their homes in accordance with their physician’s Plan of Care. You will assess speech and language needs, establish treatment plans, and collaborate with other healthcare professionals to provide patient-centered care. This role requires specialized knowledge, strong judgment, and the ability to make clinical decisions independently.
Key Responsibilities:
Provide speech-language therapy services and education to clients in their homes as per the Plan of Care.
Adhere to OASIS protocol per Medicare and State regulations.
Conduct evaluations to identify speech, language, and communication needs; assess home safety and identify necessary equipment.
Develop and implement customized treatment plans to improve client function.
Utilize specialized techniques, devices, and equipment to address communication needs.
Recommend communication aids and devices as needed.
Coordinate care with referring physicians and other healthcare personnel, contributing to the development of comprehensive treatment plans.
Participate in patient and family education regarding home health care, training them according to agency policies.
Complete and submit all required documentation within 72 hours.
Engage in case conferences, in-services, and meetings to ensure coordinated and comprehensive care.
Qualifications:
Master’s degree in Speech Pathology accredited by the American Speech-Language-Hearing Association (ASHA).
Current, unrestricted Speech-Language Pathologist license in the State of California.
At least one year of speech-language pathology experience preferred.
Current CPR certification.
Proficiency with basic computer functions (e.g., internet, email, navigation of computer desktop).
Valid driver’s license with insured vehicle per state requirements and professional liability insurance.
