Key Responsibilities:
- Patient Assessment: Perform comprehensive evaluations of patients’ speech, language, cognitive-communication, and swallowing abilities.
- Treatment Planning: Develop individualized treatment plans based on assessment findings and patients’ needs, with a focus on achieving measurable, functional goals.
- Intervention and Therapy: Provide therapy interventions for speech, language, voice, fluency, cognitive-communication, and swallowing disorders. Utilize evidence-based approaches tailored to the SNF environment.
- Collaboration: Work closely with an interdisciplinary team, including physical therapists, occupational therapists, nurses, and physicians, to ensure holistic and coordinated patient care.
- Documentation: Accurately document patient progress, treatment interventions, and outcomes in accordance with regulatory standards and facility policies.
- Family and Caregiver Training: Educate patients, families, and caregivers on techniques and strategies to support communication and swallowing function outside of therapy sessions.
- Compliance: Ensure all treatment and documentation adhere to Medicare, Medicaid, and facility guidelines.

Requirements:
- Licensed as a SLP in PA
- Valid Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA).
- 1+ year experience in a Skilled Nursing Facility
- Compassion and patience when working with elderly or disabled patients.
- Strong clinical reasoning, communication, and interpersonal skills; proficiency with electronic medical records (EMR) preferred.
