Schedule: Tuesday-Friday and Sunday 12pm-8pm

People, Incorporated is looking for an individual to join our team as a Sr. Case Manager for our Individual Supports (IS) Program. The Sr. Case Manager will assist the Assistant Division Director in day to day operations of the Individual Supports program and function as a direct care worker as needed to meet the needs of the program. The Senior Case Manager will assist in directing the work of employees and promote individual’s dignity and purpose consistent with principles of normalization while managing daily program operations and needs. This position requires 8 hours of administrative and 32 hours of direct care, including one weekend day and possible holidays.
INDIVIDUAL RELATED FUNCTIONS:
- Coordinate individual training programs in self- help skills, social aptitude, behavior management, leisure time use, and advocacy; promote individual choice, empowerment and dignity; and secure consultant services when necessary.
- Assist in relocation, when needed, including promoting preference in selection of housing and housemates.
- Assist in preparing and gathering resources needed for successful vacations and community outings.
- Provide Direct Care as needed.
- Maintain confidential files.
- Develop and create assessments for Individual Service Plans of assigned caseload.
- Maintain financial records, as needed, as well as provide training and oversight in funds management for individuals that require assistance with banking, budgeting, and paying bills.
- Assist in employment by providing transportation to/from work, review their work schedule at specific jobs, and reviewing paystubs with individuals as needed.
- Communicate with individuals, families, guardians or any other stakeholders in a professional manner at all times.
STAFF DEVELOPMENT RELATED FUNCTIONS:
- Ensure individuals feel empowered with choice, pride and ownership by supporting each individual’s specific needs.
- Assist Assistant Director with scheduling staff to work with assigned individuals, and assist with providing on-going feedback to staff on a consistent basis with monthly written performance reviews along with annual evaluations and QUICS.
- Work closely with individuals, families, and guardians to provide services that satisfy the needs of each individual.
- Address staff performance concerns in a timely fashion.
COMPLIANCE RELATED FUNCTIONS:
- Review evacuation plans for each individual/ location, ensure evacuation plans are completed for licensing requirements and copy is submitted to Individual Supports Operations Manager in a timely manner.
- Assist in conducting the QA assessment tool monthly and submit completed form to Residential Division Director in timely manner.
- Assist in providing oversight, as needed, with MAP compliance, training, and maintain familiarity with regulations and updates.
ADMINISTRATIVE RELATED FUNCTIONS:
- Assist in providing oversight to time management, coverage and overtime.
- Assist in conducting staff meetings on a monthly basis or as needed.
- Assist in coordinating licensing and other internal and/or external reviews as needed.
- Forward all individual mail from state agencies (i.e. social security benefits, mass health, food stamps) so benefits remain up to date.
- Assist in coordinating relief pool scheduling and evaluation.
- Maintain expertise in field through attendance at assigned training.
- Work collaboratively with other residential programs to provide supports as needed.
- Attend Residential team meetings as assigned.
- Review budget with Assistant Director and adhere to it.
- Assist in providing oversight about staff attendance and MITC review to ensure proper completion.
- Access and read Agency email account and Intranet at least weekly.
QUALIFICATION
- Bachelor’s Degree preferred plus three years related experience, one year in a supervisory capacity; or suitable combination of education and experience.
- Knowledge of Agency and division/program policies and procedures.
- Skill in performing counselor duties.
- Ability to manage responsibilities and others and serve as a role model.
- Ability to organize and follow-through.
- Ability to maintain current with required training/certification.
- Must be willing to work a flexible schedule to meet the needs of the individuals all hours of the day.
- A valid driver’s license is required, as well as a qualifying criminal background check.
