Job Summary and Qualifications
Under the direction of the Medical Director of the advanced heart failure program and reporting to the Director of Cardiovascular Services, the Clinical VAD Coordinator assumes clinical responsibilities for the ventricular assist device program at HCA Kendall Regional Medical Center. The Clinical VAD Coordinator is a crucial member of a complex multidisciplinary team that VAD related responsibilities include organizing and completing patient evaluations, following patients with VADs during implant, hospitalization, and during any subsequent hospitalizations, collaborating with other healthcare professionals throughout the continuum, performing clinical assessments and procedures within scope of license, making clinical judgments based upon a synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems, acquiring and managing pertinent VAD patient data and tracking of devices and related support equipment. Other responsibilities include managing the patient waitlist and coordinating the administrative and clinical aspects of VAD evaluation and placement. The Clinical VAD Coordinator will serve as an integral member of the larger interdisciplinary cardiovascular services team, contributing to excellence in patient care, research, and teaching. This role includes on call responsibility as determined by program need
Major Responsibilities:
- Collaborate with Heart Failure multidisciplinary team in providing VAD assessment and consultation when determining optimal care decisions for this complex patient population.
- Plan, perform, and evaluate activities related to implantation of ventricular devices; ·
- Serves as a resource to staff during the peri-operative phase of device implant. ·
- Provides clinical consultation and VAD-specific education to patients, families, multidisciplinary team members, hospital staff, and community based providers as needed. ·
- Assists with device tracking and ordering and maintains working relationships with device vendors ·
- Provides leadership skills such as time management, coordinating and meeting multiple patient and family needs and requests, meeting physician and team member expectations, setting priorities and networking. ·
- Collaborates with multidisciplinary teams across the care continuum to coordinate admit and discharge plans for patients post VAD implant. This will include outpatient care and clinic appointments. ·
- Facilitates the access and utilization of community resources and organizations to assist in patient's recovery ·
- Serves as the liaison and communication link between VAD patients and their families with the primary care providers, specialists, community organizations, patient care services including EMS, utility companies and volunteer programs ·
- Facilitates the readmission of the patient with a VAD, when necessary. ·
- Communicates with all involved care teams effectively to maintain positive working relationships and achieve desired outcomes ·
- Performs other duties as assigned. ·
- Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.”
What qualifications you will need:
- Advanced Cardiac Life Spt must be obtained within 30 days of employment start date
- Basic Cardiac Life Support must be obtained within 30 days of employment start date
- Registered Nurse
- Bachelors Degree
- Masters of Nursing Preferred
- 5 years cardiology and/or heart failure experience
- 2 years VAD experience
