Bachelor's degree and two years' experience in a social services related position Or completion of Master's degree in a social services related field.

- AGE RELATED AND SPECIAL NEEDS OF POPULATION SERVED
Work or Volunteer experience with individuals with mental illness, and alcohol/drug addiction. Knowledge of and process for accessing community resources. Complete essential learning and maintain related annual training requirements.
- ASSIGNED DUTIES
Counsel with emphasis on prevention and/or behavior modification. Works with individual and group to promote optimum mental health and help people live free from drugs. May help individual deal with addictions and substance abuse, family, suicide, stress management, and problems with self-esteem.
Staff will provide a broad range of services, including skill building, linking, monitoring and supporting clients with chronic mental illness and/or addictive diseases in clinic, in clinic or community settings.
Assist clients in accomplishing goals and maintaining stability through teaching daily living skills, applying for supportive services and assistance, monitoring health, financial management, provide transportation and appointment setting.
Participates in the development of treatment and discharge plans. Documents progress toward goals and objectives in each client's electronic medical record.
Works and builds relationships with other service providers and staff in support of effective client treatment, coordinating, communication and continuity of service. Keeps supervisor and other treatment team members informed of significant developments.
Duties include:
50% under the direct supervision of Licensed Staff provide 50% billable services to individuals on assigned case load. Be in compliance with service guidelines. Complete documentation with 24 hours
45% monitors service delivered are authorized, ensure caseload is accurate, and use time towards travel, scheduling and activities needed to ensure successful face to face appointment.
4% attend agency required training, supervision, meeting as scheduled by supervisor.
1% other duties as assigned, not to exceed 90 days, as assigned by supervisor. Will keep up with essential learning to perform assigned work.
