About the Job
GENERAL PURPOSE:
Provides professional comprehensive care management services to patients of the Strong Memorial Hospital and Health Home Care Management Program. Collaborates with health, behavioral health and social service providers and is responsible for the development and management of service plans for patients enrolled i n a NYS-licensed Health Home who have need for the most complex, high utilizing and costly services and may be at high risk for under-utilizing services.
RESPONSIBILITIES:
+ Under general direction and with considerable independence, provides ongoing clinical and performance supervision for Health Home Care Coordinators assigned to their team. Monitors team and individual productivity, attendance, reviews and approves leave requests, adjusts caseloads to accommodate leaves of absences and addresses performance concerns in accordance with New York State Department of Health/Office of Mental Health.
+ Provides consult and direction to the team and individual team members to ensure efficient delivery of timely, appropriate and effective services to the clients on their caseload. Oversees and completes incident reporting for care coordinators assigned to their teams in compliance with regulatory oversight agencies. The Health Home Supervisor is also responsible for auditing charts per prescribed auditing policies and on an as needed basis.
+ Responsible for making sure Health Home Care Management electronic documentation remains in compliance with regulatory oversight agencies.
+ Health Home Supervisors are responsible for conducting in-services in collaboration with the Director of Care Management on a variety of training topics including specific Health Home Care Management policies and procedures in addition to general Care Management practices, comm unity resources, etc. Team leaders will foster a supportive, diverse and inclusive work environment.
+ The Health Home Care Management Supervisor is responsible for monitoring caseload sizes, providing accurate enrollment data to the intake coordinator, and coordinating/providing caseload coverage for members of their team when out of the office.
+ The Health Home Supervisor will ensure that care management recipients are receiving care management services at the appropriate level of care. The Health Home Supervisor is expected to participate as a member of the Health
+ Home Care Management recruitment and retention team.
+ In addition to their supervisory responsibilities, under general direction and with considerable independence, Health Home Supervisors will provide Health Home Care Management services to Health Home members as part of coverage and support for their team when necessary. Supervisors in conjunction with the program Manager are responsible for planning operations in terms of short term and long-term caseload coverage, and participating in decision making that directly impacts program operations and policies. The Health Home Supervisor will represent the University of Rochester Medical Center by Establishing and maintaining cooperative working relationships with community providers and resources to obtain needed services and support for enrolled Health Home members. Tracks and monitors that the services connected to the client are delivered as planned and are effective. Utilizes community and family resources to create sustainable support systems for patients including within other programs and departments within the University of Rochester Medical Center. Reviews and discusses care plans with care managers and patients, focusing on linking individuals to clinical and social services with system and community providers. Monitors the use of costly Emergency and Inpatient services, supports patient in keeping all aftercare appointments and addressing barriers as needed. Each Health Home Supervisor is also expected to manage program specific supervisory responsibilities as assigned

QUALIFICATIONS:
+ Bachelor's Degree in an appropriate human services field required.
+ Four years of experience in providing direct services to people with serious mental illness, intellectual/developmental disabilities, alcoholism/substance abuse, or experience effectively linking people with services that address social determinants of health or an equivalent combination of education and experience.
+ Must possess and maintain a valid New York State driver's license, have a satisfactory driving record and have access to an automobile. Supervisory experience strongly recommended.
+ Must pass NYS DOH Health Home and URMC background check requirements.
+ This position requires an annual re-certification in HIPAA awareness and annual renewal of an iCARE Commitment contract.

Pay Range
Pay Range: $52,187 - $73,050 Annually
_The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job¿s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._
