Support Coordinator - DON Services

SUMMARY:  The Independent Living Supports Coordinator works to uphold excellent consumer service standards by managing resources and providing hands-on support and training for Independent Living Coordinators. All the Independent Living Supports Coordinator’s activities must reflect the Independent Living Philosophy.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

Monitor a small consumer caseload of 10-15 consumers while maintaining the support role.

Schedule on-call and pre-scheduled attendants for consumers.

Arrange for back-up attendant care.

Fill shifts when back-up care is unavailable. 

Inform ILC supervisor of problems relating to employed attendants.

Record reason for attendant call-offs or refusing work.

Verify hours worked by attendants.

Maintain weekly communication and documentation to meet consumer needs.

Visit consumers on a quarterly basis to maintain proper documentation and regulate optimum care.

Monitor consumer care in accordance with the Office of Long-Term Living by ensuring consistency and accuracy in services provided in the terms of type, scope, amount, duration, and frequency.

Assists agency staff members through individual and group conferences in analyzing problems and in improving skills and understanding to implement the independent living philosophy.

Provides support to ILC staff in completing consumer service plans and in providing services as described in consumer’s individual service plan.

Attends 100% of all new Meet & Greets and/or initial shifts.

Sets up Attendants on the HHA App.

Steps in for the ILC who is off sick or takes vacation.

Assists with On Call rotation.

Work all call offs during office hours.

Provides training, education, and direction on the quality and timeliness of all compliance documents as stipulated in agency policies and procedures and as directed by the CEO.

Participates in developing and implementing agency administrative policy.

Trains new employees in areas such as agency policy, department procedures, and agency or government regulations.  Provides in-service training for workers in areas such as new policies, procedures, and regulations. 

Completes administrative duties to ensure clear and accurate reports.  Archives all information determined important by agency operations procedures. 

Addresses and corrects issues related to compliance with IL Plans, EVV usage, and timely and accurate billing by working with ILC Supervisors and with the Compliance Department to identify and correct issues with systems, policies, and procedures.

Represents the agency in community or in interagency activities in a manner that reflects independent living philosophy.

Prepares management reports as directed.

Complies with the policies and procedures established by DON in the Employee Handbook.

Abide by the laws of the Commonwealth of Pennsylvania.

Other duties as assigned. 

AUTHORITY:  The employee may proceed with duties and responsibilities within the following limitations:  May direct resources in accordance with budget approved by the CEO; and may undertake disciplinary actions of staff in accordance with agency policy.

EDUCATION and/or EXPERIENCE:

GED or high school diploma; or up to one month of related experience or training; or equivalent combination of education and experience.  References stating that the person can competently perform this work will be required.
