Salary: $18.00 - 19.23 hourly

POSITION DESCRIPTION:
The Supportive Services Specialist will play a crucial role in connecting individuals and families to the resources and services they need to achieve self-sufficiency and improve their quality of life. This position involves direct client interaction, resource coordination, and community engagement to ensure that clients receive comprehensive and effective support.
KEY RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
Client Assessment and Support:
- Conduct comprehensive assessments to identify the needs and strengths of clients.
- Develop individualized service plans in collaboration with clients to address their unique needs and goals.
- Provide ongoing support and follow-up to ensure clients are making progress towards their goals.
- Advocate for clients to access necessary services, resources, and benefits.
- Maintain confidentiality and uphold the highest standards of client care and support.
Resource Coordination:
- Connect clients to appropriate services and resources, including healthcare, housing, employment, education, and social services.
- Maintain an up-to-date database of community resources and service providers.
Community Engagement:
- Build and maintain relationships with community partners, agencies, and service providers.
- Represent EHCEC at community events, meetings, and forums to promote our supportive services.
- Conduct outreach and education to raise awareness of available support services.
- Maintain accurate and up-to-date client records, including documentation of services provided, client progress, and outcomes.
- Prepare reports on client metrics and success stories for internal and external stakeholders.
- Utilize data to inform service delivery improvements and strategic planning.
- Associate’s Degree in Social Work, Sociology, Psychology, Human Services or related field preferred; equivalent experience may be considered.
- Minimum of 2 years of experience in social work, case management, or a related field.
- Ability to read, write, and speak fluently in English & Spanish preferred.
- Knowledge of local social service, public health, and nonprofit organizations and resources.
- Understanding of social work principles, ethics, and best practices.
- Ability to present a friendly and positive demeanor to individuals and families, staff, supportive services personnel, funders, and the general public.
- Must be organized, detail-oriented, reliable, and possess strong organizational and project management skills.
- Ability to adapt to change.
- Ability to work flexible hours, including evenings and weekends as needed.
- Valid driver's license with reliable transportation.
- Perform other duties as assigned.
- Experience working with diverse populations and addressing the needs of marginalized communities.
- Strong advocacy and client support skills.
- Proficiency in data management and reporting.
- Ability to set work priorities and to evaluate and create solutions to work-related problems.
- Cultural competence and sensitivity to the diverse backgrounds of clients.
- Ability to remain calm and maintain self-control in difficult circumstances and emergencies.
Physical Conditions:
The staff person must be able to attend meetings on nights and weekends as needed. EHCEC is a collaborative work setting. Employees pitch in to help each other with responsibilities as needed. Team members will be expected to contribute to teamwork and a sense of collaboration.
