Pay: $17.00-18.50/hour

Fellowship House is currently seeking dedicated and compassionate bachelor's-level professionals to join our team as Targeted Case Managers. This position involves helping members rebuild their skills and abilities to achieve greater independence within the community.
For those hired, we can also support you in obtaining your Certified Behavioral Health Case Management (CBHCM) credential through the Florida Certification Board!
Key Responsibilities:
- Conduct assessments to determine members' needs, strengths, and challenges.
- Develop, implement, and monitor individualized care plans in collaboration with members, their families, and other healthcare professionals.
- Coordinate services and resources, including referrals to community-based programs, healthcare providers, and social services.
- Provide ongoing support and advocacy to ensure members receive the necessary care and services.
- Facilitate communication between members, families, and service providers to promote continuity of care.
- Maintain accurate and timely documentation of the member's progress, interventions, and outcomes.
- Participate in team meetings, case conferences, and training sessions to stay informed on best practices and emerging trends in behavioral health.
- Empower members to achieve their personal goals, enhance their quality of life, and integrate successfully into the community.
Requirements:
- Bachelor's degree in the Human Services field (Social Work, Psychology, Sociology, Criminal Justice, Health Education, etc.).
- A valid Florida driver's license and access to a vehicle.
