Work Schedule (8:00 AM -5:00 PM) M-F. No Weekends! Great Benefit Package!
As a Team Coordinator, your skillful coordination of location office functions enables us to provide unparalleled care. With each patient, caregiver, and employee you impact, you breathe life into our mission, ensuring that our services consistently exceed quality measures and expectations.
+ Process approved Physician orders
+ Central Support for entry of new Physicians and Facilities.
+ Upon receipt of referral documentation and initial intake/registration, complete review of F2F documentation to ensure compliance with all required components. Follows up as needed to obtain missing/incomplete information to appropriate parties
+ Monitors and processes the referral management database on a timely basis.
+ Responsible for timely processing of designated reports, summaries, and/or coordination notes to physicians.
+ Timely processing of documentation to Physicians and Facilities required to comply with home health/hospice COP’s.
+ Complete assigned workflow tasks in a timely, accurate manner.
+ All other duties as assigned.
Qualifications
+ High School diploma or equivalent.
+ A minimum of two years customer service experience required.
+ Minimum one year of medical office related experience preferred.
+ Specialized office training strongly preferred.
+ Proficiency with a calculator and simple math.
+ Ability to produce legible, accurate, and organized records.
+ Self directed, detail oriented and organized.
+ Excellent verbal and written communication skills required.
+ Strong interpersonal skills are required.
+ Detail oriented, strong organizational and time management skills.
+ Maintains confidentiality.
+ Good computer skills, familiar with multiple software programs including Windows based programs.
Pay Range
$14.16 - $19.46 /hour
