Telehealth Nurse Practitioner - Visiting Docs

Overview:

We are seeking an experienced and dedicated Per Diem Telehealth Provider to join our growing team. This role is ideal for a licensed Nurse Practitioner (NP) with a passion for providing high-quality care through telemedicine. The candidate must have a strong clinical background and be comfortable working remotely, offering services to patients in an outpatient or long-term care (LTC) setting. 

Key Responsibilities:

• Provide telehealth consultations and ongoing care to patients within New York State
• Deliver patient-centered care and develop treatment plans in coordination with the care team
• Utilize eClinicalWorks (ECW) and healow™ platforms for telehealth visits (training provided, experience is a plus)
• Ensure timely documentation of patient encounters and adherence to clinical guidelines
• Collaborate with other healthcare providers to ensure continuity of care
• Maintain compliance with all regulatory and licensing requirements for telemedicine

Qualifications: 

• Active license and DEA registration in New York State
• Must be a Medicare/Medicaid eligible provider in New York State
• Minimum of 3 years of clinical practice, preferably in outpatient or long-term care settings
• Experience with eClinicalWorks (ECW) and healow™ systems is preferred
• Prior telehealth experience is a plus
• Strong communication and organizational skills
• Ability to work independently while being part of a collaborative team 

Schedule:

• Must have availability during regular business hours, though alternate schedules can be discussed
