WHO WE ARE: Established locally in 1911, Volunteers of America Northern California & Northern Nevada (VOA NCNN) is one of the region's largest providers of social services. Our professional staff operates more than 40 locally managed and delivered programs. Services include food and shelter, counseling, independent living and parenting skills, substance-abuse treatment and rehabilitation, and job training. Currently, VOA NCNN provides shelter or housing to over 3,500 men, women, and children every night.

PROGRAM AND LOCATION: VOA is recruiting a full-time Service Coordinator at The Village on Sage Street in Reno, Nevada. The Village is primarily designed to serve working people earning minimum wage, or those with other income sources such as social security or disability, as a low-income housing option. Full-time employees (working 30 or more hours a week) are eligible for a benefits package that includes paid holidays, vacation, health, dental, vision, life, and short-term disability insurance coverage.
Responsibilities:
Under general supervision, this position is responsible for the development of a case management plan to assist HCHV CERS (Health Care for Homeless Veterans Contract Emergency Residential Services) program participants in developing the skills necessary to stabilize in permanent housing.
OBJECTIVES/ACTIVITIES:
A. Responsible for performing daily work requirements to achieve established objectives of the department.
1. Provide records and services in compliance with HCHV CERS policies, regulatory, and funding source requirements.
2. Maintain positive relationships with program clients, Volunteers of America staff, the Veterans Administration, and all community service agencies.
3. Create and maintain case management files on each participant, including personal information, educational and job skills, short and long-term plans to become self-sufficient, outcomes of all referrals to outside agencies, and documentation of the participant’s progress and assistance in removing obstacles to set goals.
4. Complete a needs assessment with each applicant to assess housing barriers and means to mitigate the obstacles.
5. Arrange, coordinate, and/or provide direct clinical services and support for mental health stabilization and substance use treatment services using Recovery Model principles.
6. Provide crisis management as needed and mediate conflicts that may affect their housing status.
7. Complete all discharge paperwork in a timely fashion and report any changes in program participation to HCHV within a designated timeframe.
8. Develop a Housing Stabilization Plan that meets the client’s goals and assist the client in fulfilling the plan.
9. Provide resource information and appropriate case planning specific to any participant who has a physical or mental impairment.
10. Assist client in accessing and finding housing.
11. Participate in Volunteers of America and outside training sessions.
12. Provide wrap-around services with continuous engagement with aftercare services upon discharge.
13. Enter client data into all relevant databases within an appropriate and timely manner (including but not limited to: OneSite, HMIS).
14. Complete all daily and monthly data tracking and submit weekly and monthly reports to the designated VA liaison.
15. Perform other reasonably related duties as assigned by the Community Administrator.
B. Responsible for assistance with client development.
1. Assist all Veterans in follow through with obtaining tuberculosis (TB) clearance within 10 working days of admission. If TB clearance results are not available, assist Veterans in getting screened for TB via PPD or QuantiFERON or chest x-ray.
2. Act as a liaison between clients and community service agencies to meet client needs.
3. Maintain a list of all available community services and providers.
4. Assure a safe, non-judgmental environment for the clients.
5. Maintain positive, professional relationships with clients and staff.
6. Provide therapeutic and rehabilitative services: these may include clinical case management, individual or group counseling, structured group activities such as 12-step meetings, vocational training, and/or prosocial outings, linkage to and coordination with VA and/or other community-based services/resources, independent living skills, and discharge planning that begins with an assessment of housing needs upon program entry.
7. Provide a minimum of one (1) case management appointment per week that focuses on: permanent housing search and stabilization, increasing income through referral to all potential financial resources, vocational training when indicated, self-care skills, adaptive coping skills, and necessary coordination of care provided by VA and community-based medical mental health, and/or substance use treatment providers, and ongoing discharge planning that includes linkage to community resources for long-term housing success such as IHSS or Meals on Wheels. Client progress must be clearly documented in the client’s weekly case management notes.
8. Provide assistance to clients to complete their laundry on a weekly basis.
9. Provide assistance in the grocery/meal delivery on a daily and/or weekly basis.
10. Provide transportation assistance tools to clients on an as-needed basis.
11. Assist clients in securing benefits and/or income through vocational training or employment services.
12. Participate in agency and outside training sessions and certification opportunities when given the opportunity.
13. Stay current with all Federal Fair Housing Trainings and Regulations.
C. Responsible for all areas of daily program maintenance in compliance with company policies.
1. Maintain files on each participant, including all HCHV CERS eligibility information and short- and long-term plans to maintain and improve self-sufficiency. Document progress and assistance in removing obstacles to set goals. Provide routine documentation of coordination and follow-up of all areas.
2. Provide records and services in compliance with agency policies, regulatory, and funding source requirements.
3. Accept referrals and admissions Monday - Friday, 8 AM - 4 PM, and accommodate same-day admissions when the program has vacancies.
4. Maintain a minimum 80% occupancy of HCHV CERS funded beds at all times.
5. Assist, as needed, in turning over units in a quick and efficient manner to provide turnaround placement for new clients including purchasing necessary furnishing items for client move-in.
6. Maintain daily, clear, and thorough communication with the Community Administrator on all areas of responsibility listed in this job description.
7. Complete documentation and reports to comply with contractual agreement and Volunteers of America requirements, including but not limited to client demographic statistics, regulatory and funding source requirements, support services provided and referrals accepted, with copies given to the supervisor, quality assurance administrator, and the Community Administrator in an accurate and timely manner.
8. Document and report all client concerns and/or infractions that can impact the program’s security.
9. Secure approval for exceptions to standard policy.
10. Notify the Community Administrator of potential workflow problems and resource needs necessary to attain performance standards.
Qualifications:
EDUCATION AND EXPERIENCE:
A Bachelor’s Degree in Social Work or a related field is preferred. Three years of experience working with the homeless population or experience working in alcohol/drug recovery programs is highly preferred. Active or previous military service is highly preferred. This position requires demonstrated ability in record-keeping, good written and verbal communication skills, and knowledge of problems unique to women, minorities, persons with disabilities, substance abusers, low income, and the general homeless population. This position requires CPR and First Aid training within ninety (90) days of employment date and recertification as necessary. A valid Nevada driver license and the ability to meet the organization’s insurance carrier guidelines are required.
