Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens) and our Gurwin Certified and Licensed Home Care Agencies.

Our Adult Day Care Department is currently seeking a full-time Therapeutic Recreation Aide who will embody the passion and commitment required to deliver the ultimate care for our participants. You will be scheduled to work Monday through Friday from 8:30am to 4:30pm with a rotating weekend.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- High School Graduate.
- Bilingual preferred.
- Expertise in music (singing or playing an instrument) preferred.
REQUIREMENTS INCLUDE:
- Knowledge of the dementia population and dealing with related behaviors.
- Able to recognize when a resident is in pain and take appropriate actions.
- Capable of complying with governmental accreditation, professional and facility standards and regulations.
- Demonstrates ability to develop and document individualized intervention goals and plans which are registrant centered based on assessment, consistent with legal requirements and professional guidelines.
- Capable of accurate and appropriate medical chart documentation in accordance with governmental, regulatory, professional and facility standards under supervision of TR Coordinator.
- Demonstrates ability in appropriate leadership styles in planning, organizing, and conducting diversified recreation activities.
- Provides services based on TR Service Model.
- Keeps accurate statistical reports of activities offered and level of participation for person served.
- Effectively participates in the interdisciplinary care plan meeting and documents on the plan of Care under the supervision of TR Therapist/Coordinator.
- Develops treatment plans according to capabilities and interests of person served.
- Monitors, evaluates and determines effectiveness of individualized treatment plan.
- Conveys information to treatment team in a timely manner regarding person served.
- Maintains knowledge of current TR trends,techniques, methods, issues and professional legal standards.
- Keeps bulletin boards up to date.
- Maintains current lists of registrants for special programs.
- Assists in the transportation of residents as appropriate.
- Working together, showing empathy, courteous communication and respect of others.
- Capable of transporting registrants to and from recreation area.
- Actively shares knowledge, fosters and promotes dissemination of information as it relates to organizational needs.
