Type of role: Part-Time Fee for Service (Minimum 10 clinical hours weekly)
Qualifications: Active LPC, LMFT, or LCSW licensure in Connecticut
Eligible for: $1,000 sign-on bonus*
In-home Fee for Service Rates: (Fully licensed clinicians)
$60 - compensation rate per clinical session

About the position
Provide in-home non-crisis outpatient counseling to clients of our WISE program, designed to service seniors, medically compromised and intellectually disabled communities, and/or their families/caregivers. Clinicians have the autonomy to manage their own schedules, allowing them to set appointment times and arrange their work hours based on client needs and personal preferences. This includes the ability to work evenings or weekends if desired, though not required. We support a healthy work-life balance and encourage clinicians to design a schedule that fits their lifestyle while ensuring client needs are met.
Note: WISE Program Clinicians are not on call and are not case managers. This is a clinical role and we can resource connect as needed.
Who we are looking for
Fully licensed mental health therapist in the State of Connecticut (LPC, LMFT, or LCSW).
1 -3 years experience conducting counseling/talk therapy sessions with diverse client populations.
Previous experience working in the medical field, with seniors and/or intellectually disabled clients *do not have to have experience with all 3 to apply.
Passion for in-home work.
Ability to work collaboratively in a team-oriented environment.
Advanced proficiency in conducting intake assessments and formulating treatment plans.
Strong written communication and interpersonal skills with the ability to provide and maintain documentation within the appropriate time frames.
Have a valid driver's license and access to a properly insured vehicle for work or reliable transportation.
What you’ll do:
Under the direction of a senior member of our Clinical Team, you will:
Assess the mental and behavioral health status of referred clients with mild to moderate mental health conditions and/or substance abuse issues and Conduct standardized counseling/talk therapy sessions for Stokes Counseling clients
Perform intake/biopsychosocial assessments to evaluate clients' mental and behavioral health status.
Formulate differential diagnoses along with short-term and long-term goals.
Provide and maintain written documentation of intake, assessments, progress/contact notes, interventions, treatment plans, and termination notes within the designated time frame.
