Position Summary:
The In-Home Therapist is responsible for providing intensive home-based therapy to clients and their families as part of a multidisciplinary team. This role involves working with a caseload of 20-24 clients, collaborating closely with three Qualified Mental Health Specialists (QMHS) to deliver individualized, family-centered care. The In-Home Therapist will provide therapy, develop treatment plans, and assist in crisis intervention, all while ensuring compliance with state regulations and agency standards. Candidates must hold a current LSW, LPC, or LMFT license (or higher) in the State of Ohio. A Bachelor's degree in Counseling, Social Work, or Marriage and Family Therapy is required, with a Master’s degree preferred.
Essential Duties and Responsibilities:
Therapeutic Services:
- Provide individual, family, and group therapy sessions to clients in their homes.
- Conduct comprehensive assessments to inform the development of individualized treatment plans.
- Utilize evidence-based therapeutic techniques to address mental health, behavioral, and emotional issues.
- Collaborate with clients and families to set measurable goals and monitor progress.
- Offer crisis intervention services when necessary, ensuring the safety and stability of the family unit.
Case Management:
- Manage a caseload of 20-24 clients, ensuring that all clients receive appropriate care based on their individual needs.
- Coordinate care with three Qualified Mental Health Specialists (QMHS), ensuring seamless delivery of services.
- Conduct home visits as necessary to assess the environment, build rapport, and provide services directly to clients and their families.
- Monitor client progress, adjust treatment plans as needed, and document all services provided in a timely and accurate manner.
- Assist clients in accessing community resources, including psychiatric care, medical services, and educational or vocational programs.
Collaboration and Teamwork:
- Work closely with the multidisciplinary team, including QMHS, to ensure that all aspects of the client’s treatment plan are effectively implemented.
- Participate in regular team meetings, case consultations, and supervision sessions.
- Communicate effectively with other providers, schools, and community resources involved in the client’s care.
- Advocate for clients' needs and ensure that all services provided align with ethical standards and best practices in mental health care.
Compliance and Documentation:
- Ensure all clinical documentation, including assessments, progress notes, treatment plans, and discharge summaries, are completed in compliance with state regulations and organizational standards.
- Maintain confidentiality and adhere to all HIPAA regulations.
- Participate in ongoing professional development to maintain licensure and stay current with industry best practices.
Minimum Qualifications:
- Education:
- Bachelor’s degree in Counseling, Social Work, Marriage and Family Therapy, or a related field (Master’s degree preferred).
- Licensure:
- Must hold an active LSW, LPC, or LMFT (or higher) license in the State of Ohio.
- Experience:
- Prior experience in providing therapeutic services to children, adolescents, and families in a community or home-based setting is preferred.
- Familiarity with trauma-informed care, family systems theory, and crisis intervention techniques.
- Skills:
- Strong clinical assessment and diagnostic skills.
- Excellent interpersonal, communication, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records (EHR) systems and timely documentation practices.
Physical Demands and Work Environment:
- Ability to travel locally to client homes and community locations as required.
- May be required to work flexible hours, including evenings or weekends, to meet the needs of clients.
Compensation details: 55000-75000 Yearly Salary
