Job Summary:
The Thoracic Nurse Navigator will facilitate a timely, integrated and coordinated approach to the care of patients with thoracic disease. Coordinates care for patients throughout the continuum of care from diagnosis and throughout treatment. Uses a multidisciplinary approach and acts as a liaison between the patient, staff and referring physician offices as well as ancillary departments of the hospital system as well as outside agencies. The Nurse Navigator will advocate for and guide the patient/family through the complexities of the healthcare process, working to eliminate barriers that might otherwise adversely impact patient care/outcome and will help to coordinate the efforts of the medical team to facilitate continuity of care and enhance patient’s satisfaction.
Duties and Responsibilities:
Collaborates with the interdisciplinary team in guiding the patient and family regarding diagnostic and treatment options. Provides emotional support specialized patient care, support, and educational needs. Assumes the role of care coordinator and patient advocate throughout the process.
Coordinates tests, treatments, referrals, and appointments as necessary to meet the physical, spiritual, and psycho-social needs of the patient and family. May provide clinical support and patient care during procedures • Screen new patient referrals and ensure appropriate testing has been completed prior to visit, help coordinate imaging retrieval and ensure they are uploaded for visit
Coordinate tumor/nodule board including documentation and patient follow up
Ensure patients complete pre-operative work up and clearance and that post op orders are in place
Coordinate with OR and office schedulers to optimize patient throughput and new patient scheduling
Clinical triage of patient phone calls
Coordinate care between Kent and BWH – (insurance auths, letters, path reviews, imaging sent/reviewed, emails to team at BWH, post op coordination etc.)
Point person for nodule clinic for Thoracic team, working closely with Pulmonary Nurse Navigator to optimize program
Thoracic team point person for manometry testing
Post operative contact with patients
Preserves and protects patient rights to confidentiality.
Performs other related duties as requested or directed.
Requirements:
Bachelor's degree required.
Minimum 3-5 years experience.
Current Rhode Island RN license.
CPR Certification required; Nurse Navigation Certification preferred.
