Job Summary:
The Independent Supports Broker Mentor/Mental Health Professional (Social Worker Manager) oversees and coordinates the implementation of Person-Centered Planning and the principles of Self Determination in the lives of people supported by Community Living Services of Macomb County. The Independent Supports Broker Mentor/Mental Health Professional (Social Worker Manager) plays a key role in the recruitment, credentialing, and training of Independent Supports Brokers. The Independent Supports Broker Mentor/Mental Health Professional’s (Social Worker Manager) responsibilities include facilitation of the initial intakes/annual assessments, the facilitation of initial Person Centered Plans/annual Person Centered Plans, oversight, training and mentoring of the Independent Support Broker and Clinical network, community outreach, obtaining and utilizing stakeholder input and assuring quality services and satisfaction with services while keeping the people supported by CLS and their needs the primary focus of all activities.
The Independent Supports Broker Mentor/Mental Health Professional's (Social Worker Manager) responsibilities include maintaining communication and collaboration with Macomb County Community Mental Health, processing new referrals, completing intakes, providing interim support brokering services, and linking individuals served with resources as needed. This individual will maintain a data base of individuals served through this division and will act as a Liaison between funders, subcontracted service providers, families served, and other stakeholders. The Macomb County Services Coordinator/Mental Health Professional’s responsibilities include coordinating, tracking, and developing Macomb County specific training.
Reporting Relationship: Reports to the Macomb Services Coordinator
Essential Duties and Responsibilities:
- Ensures the coordination and provision of Intake functions, including eligibility determinations, coordination with MCCMH’s Managed Care department, initial assessments, provision of emergency needs, required authorizations and releases and crisis planning.
- Provides all enrollees with quality customer services and culturally respectful supports.
- Facilitates the initial Person Centered Plan for all individuals coming through intake. Facilitates the annual Person Centered Plan for the individuals currently enrolled. Coordinates with MCCMH’s Managed Care department for authorization requests within the Person Centered Planning process, budget development and management, and liaison activities to pursue identified resources, entitlements and supports.
- Reviews and processes Person-Centered Plan Periodic Reviews and Addendums authored by Independent Supports Brokers to assure quality and timeliness.
- Assures continuity of support to persons with developmental disabilities by reviewing required reports and summaries of service for funding sources. Reports individual and agency successes, outcomes and needs to funding sources and internal/external customers and stakeholders.
- Provides ongoing education and training to Independent Supports Brokers to assure services are provided in line with MCCMH, state and federal expectations.
- Provides leadership and direction to Independent Supports Brokers and Network Providers reinforcing organizational principles and setting the tone and expectation for quality service delivery.
- Educates and empowers persons served and families. Provides resources related to provider interviews and selection.
- Collaborates with traditional support systems to develop new and innovative methods of service provision that address gaps in service delivery and meet identified needs.
- Reviews and develops systems that assure adequate delivery of services to individuals with developmental disabilities. Meets with Network Providers, Community Mental Health Administrators, and various others as needed to ensure effective means of continuing quality services.
- Performs quality management functions. Develops and monitors systems to assure delivery of quality services. Prepares corrective action plans to address any patterns/trends that reveal need for improvement or areas of deficiency cited following internal and external reviews. Assists with the development, coordination, and monitoring of the CLS Compliance Plan.
- Responsible for Community Outreach activities including presentations to groups and individuals regarding Community Living Services and Self-Determination principles and practice.
- Recruits, develops, and maintains a strong provider network. Chairs committee activity as appropriate. Attends and participates in committees as assigned.
- Provides Support Broker Services for people served via the CLS/Macomb County Contract as needed.
- Monitors individual and aggregate authorizations. Oversees process for communicating service authorizations and expiration dates to network providers.
- Assures continuity of support to people served. Monitors and oversees service utilization. Identifies any areas of underutilization and proactively acts to assure all needs are being met.
- Works closely with clerical and accounting staff to assure accurate invoicing and data reporting occur within the contractual outlined timeframes.
- Reports individual and agency successes, outcomes, and needs to funding sources, CLS/OC Management, and internal/external customers and stakeholders.
- Leads in the development and implementation of Macomb County specific policies and procedures. Assists Director with development and maintenance of the Handbook of Services. Participates in the development and implementation of policies and procedures.
- Maintains confidentiality of personal and agency information. Performs special projects and other duties as assigned by supervisor. Accepts on-call responsibility. Works adjusted hours as needed.
Qualifications:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required.
Education and/or Experience:
- Minimum of Master’s Degree in a Human Services area (master’s level social worker, psychologist, licensed professional counselor, licensed marriage and family therapist).
- Two or more years experience working with persons with developmental disabilities. Experience should demonstrate effectiveness in developing and implementing creative support solutions.
- Experience working with people with challenging behavioral issues and mental illness.
- Experience providing leadership and oversight for clinical teams working in challenging environments.
- Experience and knowledge of gentle teaching principles and implementation of behavioral treatment plans.
- Computer skills required, including word processing, spreadsheets, flowcharts, and use of internal and external e-mail.
- Extensive experience working with families.
Special Knowledge:
Knowledge of the principles of Person-Centered Planning and Self-Determination. A clear understanding of the roles and responsibilities of the Independent Support Broker and the Support Circle is necessary. Desired attributes include creativity and a willingness to develop knowledge bases and share information with peers. Good interpersonal and negotiation skills.
Knowledge of Macomb County Community Mental Health policies and procedures.
Physical Demands:
This job is essentially an office position with some field work required.
Extensive scheduled and non-scheduled travel is required in executing essential duties and responsibilities of this position. Employees must provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle registration and insurance in compliance with State of Michigan requirements are all continuing conditions of employment. If driving a vehicle other than the one listed on file with the Human Resources Department, employee is responsible for ensuring that the vehicle is registered, insured and meets all other State of Michigan motor vehicle requirements and providing documentation as requested. Use of an alternative driver is prohibited.
