Transitional Housing Case Manager
Position Summary:
Counsel and support young adults (THPP-NMD 18-21 and THP+ 21-24) in developing basic like skills, identifying community resources, and planning for their own self-sufficiency.
Essential Duties:
- Acquires, maintains, teaches and demonstrates knowledge of Continuum of Care, Shared Core Practice Model, evidence-based practices, and trauma-informed care and maintains a professional environment that is consistent with Title 22, licensing regulations, and established agency policies
- Assist young adults to develop Transitional Independent Living Plan (TILP), Shared Living Agreement (SLA); and other required plans depending on program.
- Provide individual and group instruction in basic life skills.
- Provide individual and group instruction in job-readiness.
- Assist young adults in identifying and accessing resources in their home community.
- Assist young adults in obtaining documents such as birth certificates, ID cards, etc.
- Instruct and support young adult’s access to public transportation
- Assist young adults in obtaining safe and appropriate housing.
- Assist and provide education to young adults about higher education opportunities
- Encourage and support young adult’s transition to independence in accordance with their TILP.
- Meet with young adult for face to face case management services or schedule regular contact meetings as outlined per agency and Community Care Licensing standards
- Present psychosocial needs, strengths and concerns of young adults during supervision.
- Report all serious incidents, allegations, grievances, and sensitive situations to Transitional Housing Program Director immediately.
Minimum Qualifications
BA/BS in related field required, as is a minimum of two years working directly with transitional age young adults. Working with special needs populations such as pregnant/parenting young adults, young adults with mental illness or behavioral problems, system-involved young adults, and/or immigrant/refugee populations strongly preferred.
Additional Requirements:
- Case Managers assigned to mono-lingual clients must be proficient in the young adult’s primary language (if Spanish) or must be able to work effectively through interpreters.
- Must possess and maintain a valid, unrestricted California driver’s license and provide safe, reliable transportation of young adults to necessary appointments and activities.
- Must be able to successfully complete and maintain required certifications and training including, but not limited to First Aid/CPR.
Salary Range: $50,232 - $55,255
