Position Summary:
The Transitional Housing program at Veterans Place provides up to two years of residential Support for Veterans who are homeless or at risk of homelessness. The Intake Coordinator is the first face to face contact that a veteran entering Veterans Place will have. This person will work as the center piece of the Transitional Housing team and Coordinate services for residents via other Veterans Place staff and partner agencies, including The Veterans Administration.
Position Responsibilities:
Service Activities:
· Act as the point of contact for referrals to the transitional housing program.
· Accept referrals to transitional housing from the VA, internally and from the community
· Conduct assessment interviews with veteran clients
· Assist veterans with the creation and implementation of the initial Individualized Service Plans (ISP) which incorporate housing, employment, educational and income/financial goals.
· Maintain the agency's "Bed Board" that tracks referrals and admissions
· Ensure that all parties are kept up to date on the "Bed Board" and any changes that occur
· Collaborate with the Director of Clinical and Recovery Services to determine appropriate placement in Transitional Housing programming
· Assist with coordination of resources within the continuum of care
· Assist and monitor all community activities within scope of program
Clinical Activities:
· Act as first point of contact for people seeking treatment services
· Track calls for service and initial screening of people seeking services
· Complete level of care assessments to determine appropriateness for services provided by Veterans Place
· Instruct educational seminars as assigned
· Attend Team Support Meetings, Recovery Plan Update meetings, and clinical supervision as scheduled
· Provide crisis intervention
· Facilitate Specialty groups as assigned
· Work cooperatively with the Director of Clinical and Recovery Services to manage referrals and follow-ups with Veterans referred
· Work cooperatively with Fiscal Administrator to ensure that billing is timely and correct
· Prepare case presentation for assigned clients at Team Support Meetings and ISP update meetings
· Determine resources needed to meet the individualized needs identified in the assessment process and assist with the coordination of community support activities
· Ensure that all Veterans Place, VA, county, MCO, contractual and state regulations and expectations are met and that the program is prepared for review with limited notice
· Perform case finding functions and outreach, identifying and screening Veterans for psychosocial needs
· Conduct psychosocial assessments and develop initial ISP's in collaboration with the Veteran and other provider organizations including the Veterans Administration
· Coordinate referrals of Veterans from the Veterans Administration and other community-based providers for additional services based on needs and eligibility (this includes expanding the existing referral network as necessary)
· Maintain case records in accordance with program guidelines and professional requirements.
· Implement legal and ethical standards of service delivery, privacy and confidentiality, complying with all applicable regulations
Program Development:
· Take part in community events related to Veterans employment.
· Oversee the delivery of services within the scope of this program and make recommendations to the Director of Clinical and Recovery Services for further development and refinement of the program
· Work closely with the VA Grant and Per Diem Liaison to ensure a high quality of Services
Communication:
· Prepare reports as requested by other Veterans Place and Veterans Administration staff.
· Participate in administrative, staff, clinical, and in-service activities and training as required.
· Prepare written reports and complete documents as requested by the Director of Clinical and Recovery Services
· Communicate policies and the rationale behind them to residents.
· Inform other agency staff members of relevant matters related to the Transitional Housing Program.
