Position Summary:
We have an exciting opportunity to join our team as a Transitions of Care Coordinator, Kimmel Ambulatory Care Pharmacy.
In this role, the successful candidate A Transitions of Care ToC Coordinator is responsible for helping to establish correct medication history at hospital admission Duties include obtaining Gold Standard medication histories appropriately documenting medication histories in our health systems Electronic Health Record Epic and collaborating with the pharmacy team and other healthcare providers as needed Other duties include coordinating onsite with other team members from the transitions of care team medication to bedside team nurses clinical pharmacists outpatient pharmacists and independent practitioners
Job Responsibilities:
- Demonstrate knowledge of the organization rsquo;s Service Standards and incorporate them into the performance of duties
- Work with the emergency department pharmacy team to establish candidates for Medication Reconciliation
- Adhere to guidelines established for the medication reconciliation processes nbsp;
- Interview patients and caregivers to obtain accurate medication history
- Confirm medication histories with outpatient pharmacies nbsp;
- Appropriately document medication histories in the EMR
- Participate in collecting and analyzing data for any metrics developed for the ToC program
- Demonstrate appropriate collaboration with the pharmacy team providers and other healthcare professionals
- Maintain a patient oriented workflow clear times frames for process
- Maintain reliable attendance and punctuality record; adhere to departmental and organizational policies
- Perform all other responsibilities as directed
Minimum Qualifications:
To qualify you must have a Bachelors Excellent communication skills Pharmacy technician NYS registration 3 years recent experience in a busy pharmacy setting Excellent attendance in work environment Must obtain ASHP Medication Reconciliation certification within 6 months of hiring date
Preferred Qualifications:
Previous experience conducting medication histories in a hospital environment 5 years of experience as a pharmacy technician or pharmacy support staff in a busy retail pharmacy Lead pharmacy technician in a retail pharmacy ASHP Medication Reconciliation certification

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $81,325.15 - $135,541.73 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
