Job Description
Under the general supervision of the Transplant Program Manager, provides professional social work to facilitate care for transplant patients. Responsibilities include; care coordination of the bio-psychosocial needs of transplant patients and provides leadership in the development, implementation and coordination of treatment plans. In collaboration with other health care professionals, Master Level Social Workers provide a variety of comprehensive services in order to maximize quality, cost effective health care along the continuum of care appropriate to the transplant patient’s needs.
Provides transplant social work services for facilitating solutions to the medically related psychosocial/environmental problems of patients and families. Ensures the continuity of patient care by implementing interventions which may include initial assessment, identifying mental health disorders, brief behavioral health interventions, information and referral, during hospitalization and/or outpatient treatment. May provide educational support to staff. Maintains positive working relationships with vendors and external agencies to access services for patients.
