Transplant Program Assistant—Phone Triage and Scheduling
Job Description
The Transplant Program Assistant (Phone Triage and Scheduling) reports to the Transplant Program Manager of the Transplant Center. Under general supervision, the Transplant Program Assistant helps to establish, coordinate and perform all administrative duties required to support the transplant team. The position operates as the liaison between patient and the Transplant Center’s clinical staff in communicating relevant information by telephone and written correspondence. Additional responsibilities include maintaining the transplant data base and accurate data entry, ongoing training of new staff and keeping up to date with the regulatory changes in UNOS and CMS
Job Requirements
High School Diploma or equivalent (GED) with a minimum of three years of work experience in a data entry/administrative position; or an associate degree with two years of work experience in a data entry/administrative position: or a bachelor’s degree with one year of work experience in a data entry/administrative position. Ability to establish and maintain cooperative working relationships with a wide range of groups and individuals. Extensive knowledge of office management, policies and practices in a complex environment required. Ability to establish cooperative working relationships with diverse groups and individuals, medical staff, and other health care disciplines and understanding of the consultative process.

For every 100 additional pounds, assistance will be required from another healthcare worker. 20lbs of force is needed to push a 400lb patient in a wheelchair on carpet. 25lbs of force is required to push a stretcher with a patient with one hand. Ability to maintain vision 20/40 corrected (C). Ability to see and recognize objects close at hand or at a distance (C). Ability to match or discriminate between colors (C). Ability to determine distance/relationship between objects; depth perception (C). Good peripheral vision capabilities (C). Ability to maintain hearing acuity, with correction (C). Job Responsibilities
60% Provides administrative support to the transplant team throughout the transplant phases; pre-transplant, intra-transplant, waitlist and post-transplant. Obtain, coordinate, and maintain transplant patient records. Work in conjunction with patients and referring providers to assist patients in accessing transplant resources and care.
Success Criteria:
Demonstrates compliance with designated workflows as measured by chart audits and EMR reports.
Document all communication with patient and outside agencies in the EMR.
Ensure all regulatory elements of transplant care are adhered to related to patient communication.
Works in conjunction with the various services throughout MUSC to ensure that the care of the transplant patient is efficient, consistent and customer-friendly.
Communicate professionally with fellow co-workers to provide consistent and cost-effective care to the transplant patient and family.
Exhibits a spirit of teamwork.
10% Actively participates in quality assurance activities.
Success Criteria:
Participates in quality improvement processes to improve care and outcomes.
Communicates with team members to achieve objectives.

Clinical Transplant Coordinator 11 (Care Coordinator)Exempt or non Exempt
Job Description
The Clinical Transplant Coordinator II reports to the Transplant Programs Manager of the Transplant Center. Under general supervision, the Clinical Transplant Coordinator II develops and uses advanced clinical management, consultation, education and research to promote quality care for the specific transplant patient populations. Provides growth in a clinical knowledge through research-based practice with peers. This position is responsible for the ongoing training of new staff and keeping up to date with the regulatory changes in UNOS and CMS.
Job Requirements
A Bachelor's degree in nursing and three years’ experience as a registered nurse. Transplant nursing experience preferred. Current South Carolina licensure or compact state required. An ability to establish cooperative working relationships with diverse groups and individuals, medical staff and other health care disciplines. Ability to establish and maintain cooperative working relationships with a wide range of groups and individuals. Highly developed analytic skills, interpersonal skills and organization skills. Must have emotional/mental ability to perform effectively and efficiently under pressure and during stressful or emergency situations. Must maintain the highest standard of professional conduct and confidentiality. Current Basic Life Support (BLS) required, either a certification from an American Heart Association (AHA) BLS for Healthcare Providers (or AHA recognized equivalent) or an American Red Cross CPR/AED for Professional Rescuer and Healthcare Provider. Certified Clinical Transplant Coordinator (CCTC) is preferred.

Initiate and develop case management plan to provide optimal care during all phases of transplant, ensuring patient has adequate resources to optimize outcomes.
Act as a consultant/collaborator with nursing staff and physicians for the care of the transplant patient.
Perform effective patient, family and staff education in the course of triage, treatment or admission.
Demonstrate ability to show complete documentation for all actions/interactions involved in patient care.
15% Initiates and/or actively participates in quality assurance activities. Evaluates patient population for trends and risk factors and recommends innovative strategies to improve quality of care. Success Criteria:
Actively participates in activities which enhance career development and meets the organizational goals of MUSC.

Additional Job Description
A bachelor’s degree or high school diploma and four years of relatable work experience is required. Previous experience in a secretarial/clerical position is preferred. Basic keyboarding skills are required; medical terminology is preferred. Applicant must communicate effectively both verbally and in writing.
