- Prescreening Document
- Covid Test 72 hours prior to start date with results issued 48 hours prior to start date
Job Description:
- Fill out and attach the prescreening document upon submitting an application.
- Ensure the resume and license verifications are attached.
- Provide three references.
- Complete the following background checks:
- County Criminal Check (7 years for all counties lived and worked)
- Federal Criminal Check (7 years or more)
- Multi-State Criminal Search (7 years or more)
- Nationwide Sex Offender Search (7 years or more)
- HHS/OIG Search
- GSA Search
- Social Security Number Trace
- 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
- SAM Search
- CORI Massachusetts Background Check
- Complete 8 hours of Dementia Training.
