**Job Title:** Rehabilitation Therapy Assistant

**Certifications:** CPR (Fundamental Facilities)
**Must-Have:**
- Completion of prescreening documents
- Submission of resume and license verification
- Covid test 72 hours prior with results issued 48 hours before the start date
**Job Description:**
- Provide support in implementing occupational therapy programs under the direction of an occupational therapist
- Assist patients with therapeutic activities and exercises designed to improve their independence and quality of life
- Maintain accurate records of patient progress and report changes to the supervising therapist
- Ensure adherence to all safety protocols and confidentiality requirements
- Engage with multi-disciplinary teams to provide holistic care for patients
- Complete required credentialing checks, including background and criminal history verification
**Credentialing Checklist:**

- County Criminal Check (7 years for all counties lived and worked)
- Federal Criminal Check (7 years or more)
- Multi-State Criminal Search (7 years or more)
