Travel/Contract Registered Nurse - Home Health and Hospice - Up to $67/hr - Connected Health Care, LLC

Connected Health Care is seeking a dedicated and experienced Registered Nurse (RN) for a local travel contract position in Home Health and Hospice near Taos, NM. Job Description:

• Deliver comprehensive nursing care to patients in their homes and hospice settings.
• Conduct patient assessments, develop care plans, and monitor patient progress.
• Administer medications and treatments as prescribed.
• Provide patient and family education on disease management and end-of-life care.
• Utilize wellsky/kinser software for documentation and care planning.
• Coordinate with other healthcare professionals to ensure cohesive patient care.
• Maintain accurate and detailed patient records.

Education and Certification Requirements:

• Active RN license in New Mexico
• Home Health & Hospice experience required
• Proficiency with Wellsky/Kinser software
• Current BLS certification
