As a Hospice RN Case Manager, you'll play a vital role in planning, organizing, and directing hospice care in patients' homes and communities. Key Responsibilities:
- Conduct comprehensive assessments of patients and families to determine hospice care needs.
- Develop, initiate, and revise care plans based on patient needs and status.
- Administer medications and treatments as prescribed by physicians.
- Offer counseling and support to patients and families, including education on self-care techniques.
- Coordinate patient care and communication with interdisciplinary teams.
- Participate in on-call duties and ensure necessary equipment and services are available.
Key Qualifications:
- Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and Active CPR certification.
- Valid driver's license, personal automobile, and insurance coverage.
- One year of hospice care experience is preferred.
- Case Management experience and supervision of hospice aides is a plus.
- Excellent observation, communication, problem-solving, and nursing skills.
- Ability to perform physical tasks such as lifting, positioning, and transferring patients.
- Effective social interaction, communication, and organizational skills.
