- Work to meet and uphold standards of practice in home care settings

The open position is for a Home Care Social Worker covering the Blue Hills Team (Dedham, Hyde Park, Roslindale, West Roxbury) and the Paul Revere Team (Dorchester, Jamaica Plain, Mission Hill, Roxbury)
