(Y/N) – YES 3.5 FTEsSkill Set Most recruited for: (Travel RNs, local CNAs and sitters)Case Manager: Provides Case management service for both inpatient and observation patients assigned.Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team.Evaluates progression of care using InterQual throughout the episode of care.Social Worker: Works collaboratively with RN case managers and the multidisciplinary team, performs assessments and develops appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.Engages in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.
