Strong discharge planning exp REQUIRED
Must have recent strong acute care hospital discharge planning experience.
Conducting patient initial assessments
Acute Care hospital experience
EPIC EMR documentation experience
Active CA RN License
Must have BLS from American Heart Association
Shift note: Monday through Friday - 0800 to 1630.(no weekend)
