About the Job
- Valid Certified Diabetes Care and Education Specialist credential in addition to: Valid license in good standing in the State of Alaska as a Registered Professional Nurse.
- Preferred Basic Life Support (BLS) certification.
Certification Details
- Position Summary: This is a TDY grant funded position. TDY Registered Nurse Case Manager/Clinical Diabetes Educator (certified) is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting. Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This position will assist in the planning, coordinating and providing of diabetes-related health promotion activities. Patients and activities may involve multiple village trips per month. Works closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care. Assists in maintenance of the Diabetes Registry and grant requirements. Participates in all program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events. Participates in department planning and goal setting to achieve excellence in patient care and partnership with our tribal leaders. Believes in and promotes Napartet and all corporate endeavors. Performs other duties as assigned or required.
Job Details
- Minimum Education: Must have completed a program of education in nursing from an NLN approved institution. BSN required, MSN preferred.
- Minimum Experience: Two (2) years experience in a clinical health setting.
- Demonstrated experience collaborating with a variety of disciplines for project completion.
- Background in Diabetes Education required.
Job Requirements
- Occasional long working hours.
Schedule Information
- This at times does require village travel.
- Work in a typical office, hospital, and village clinic setting.
- Comprehensive knowledge of utilization and electronic health records.
- Ability to operate diabetes care medical equipment such as glucometer, ht./weight scale, etc.
- Travel to villages via small plane/sub-arctic conditions, moderate lifting of equipment.
