Job Details

- Minimum of 2 references covering 1 year of employment total within the last three years within a comparable specialty/setting that they are booked
- Application must include at least the last 5 years of employment
- Physical Exam within 1 year of start
- Fit Mask within 1 year of start
- Tetanus/Diphtheria/Tdap within 10 years
- IGRA/TSPOT, QGold, or 2 PPDs within 12 months of start
- 2 MMR Vaccines or Positive Titer
- Hepatitis B documentation
- Varicella documentation
