Position title Resident Care Manager (RCM)

Reports To DNS/Administrator
JOB SUMMARY:
The Resident Care Manager (RCM) has 24 hour accountability for the care of all residents in Blue Mountain Care Center (BMCC). The RCM’s name will appear on the resident’s medical record and on the care plan. The RCM collaborates with all members of the health care team to develop and maintain a high quality of nursing care for his/her residents. The RCM works under the direction of the Director of Nursing and/or Administrator.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
•Adheres to Oregon State rules and regulations concerning delivery of resident care and assures that nursing services are provided and documented.
•Assures that resident admission status is assessed and documented within 8 hours of admission.
•Assures that a plan of care for nursing services is developed and documented within 24 hours of admission, and completed within 14 days.
•Assures that residents’ condition is updated and reviewed when residents’ condition changes and no less than quarterly both in resident record and care plan.
•Has good working knowledge of State and Federal Rules and Regulations.
•Completes comprehensive assessment within 14 days of admission.
•Assists during annual State Survey and other surveys as requested.
•Is aware of delinquent physician visits as reported by Medical Records and assists with communicating the need for visits to physicians as necessary. Assures standing orders are replaced in the charts annually.
•Demonstrates observation skills for obvious and subtle changes in resident’s condition.
•Accurately assesses resident physical and psychological status in collaboration with the charge nurse.
•Reads 24 hour report and Charge Nurse Book and checks resident’s records for documentation.
•Identifies resident illness (via labs and presenting symptoms, etc.) and utilizes necessary resources for unfamiliar conditions.
•Reports changes in resident’s condition to appropriate personnel (charge nurse, DNS, physician) and documents.
•Responsible for initiating and discontinuing Alert Charting or assuring it is done.
•Alters care plan to reflect changes in resident conditions.
•Provides resident and family with explanations of the conditions as necessary.
•Implements physician orders in a timely manner with understanding of purpose of order as it relates to the resident’s condition.
•Coordinates provision of services with other health care providers (physician, PT, SS, Activities, etc.).
•Applies problem solving principles in care delivery process.
•Investigates incidents, accidents and errors; initiates corrective action as necessary.
•Conducts weight loss, care plan or other meetings as needed, as well as monthly nursing QA.
•Assists in orientation of new staff.
•Provides input with employee performance evaluations as needed.
•Recognizes legal responsibilities and functions within scope of practice.
•Demonstrates professional responsibility of conduct and attendance record.
•Provides input to change process and supports DNS and Care Center in process.
•Covers for DNS in absence (taking call, attending or holding meetings, supervising staff, covering the schedules).
•Delegates nursing functions and tasks to RN’s and LPN’s within their scope of practice.
•Completes the MARS, TARS, ADL’s and 60 day orders appropriately and in a timely manner.
•Assures accurate completion of resident chart in collaboration with Medical Records person.
•Interprets and adheres to established policies of Care Center.
•Reports all problems, needs and concerns to the DNS.
•Will perform other duties as assigned
•Will assist in the supervision of staff.
•Will cover the floor in the absence of the charge nurse.
•Will cover every other weekend on the floor as the charge nurse as needed to meet the state mandated requirement of (1 RN in the facility for a minimum of 8 hours every day).

? Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards.
? Reports to work on time and as scheduled.
? Wears identification while on duty.
? Attends annual review and performs departmental in-services.
? Represents the organization in a positive and professional manner.
? Attends committee and management meetings, as appropriate.
? Complies with all organizational policies regarding ethical business practices.
? Communicates the mission, ethics and goals of the facility.
EDUCATION REQUIREMENTS:
•Required: Graduate of an accredited program of nursing education holding a current Oregon RN license
•Preferred: Bachelor’s Degree in Nursing or Nursing Administration
CERTIFICATION/LICENSURE REQUIREMENTS:
•Licensure in the State of Oregon
•Experienced in the care of the geriatric resident as evidenced by completed courses and clinical practice. RCM course within 6 months of hire.
•Certification from the Oregon State Board of Nursing
•BLS certificate
