1 yr of Transitional Care -36 or 48 hrs a week LTC/SNF , BLS (AHA) required, Ratio 1:8 max. must have exp with Specialty Beds, Philips Monitor Vital Sign Machines, Overhead lifts, Sara Steady, Standing Scale, Bladder Scanner, Hover Mat, Sage Mat, Synergy Air beds, Stryker beds, stretchers, wheelchairs, walkers, slide boards.-Must have recent Acute Care Experience, Charting / Epic ExperienceType of Patients on Unit: Mix of Medical and Surgical, Medically ready for Discharge.$66 OT rate and $3278 for 48 a week.
